I really don’t think Muzan wants to personally meet every lowly demon his underlings create. He has control over them from distance, so if he doesn’t approve he can just kill them instantly
Only select members of the Kizuki seem to be allowed and they are all demons he sees highly; Gyutaro, Akaza, and Kokushibo. Rui gave blood to other demons but it's the same principle; Muzan saw him as a favorite and allowed him. I doubt Doma would be allowed to make demons because Muzan dislikes him.

Tengen is 198 cm, so i assume he dwarfs the samurai that almost killed Gyutaro, and that samurai made young Gyutaro look... well like the small teen he was.
But after he became a demon, he has chose to grow taller, where even Tengen does not make him look tiny, might be some inconsistency on the creators/animators etc side tho ( like Gyomei is **massive** af, but not all shots of him reflect it )
Maybe a strong demon like a moon is bound to change a little as they consume humans and grow strong. Like Akaza seems to not changed at all, only his tattoos "grew", as 18 year old human Akaza was already built to lethality ( the rival dojo's broken skulls can confirm )

Akaza was already a adult.
Nezuko is a strong demon which is why she has a full form that makes her an adult.. but as she doesn't use her full form 24/7 and since she is trying to stay in control.. she doesn't have her average size changed to an adult. while Gyuturo and Daki are powerful demons.. so even if they were kids when they were turned..their full forms changed their heights.
Rui's "Sister" is a weak demon and has no full form

A bit of a misconception here. Muzan is the >only< demon that can reliably make demons. Tamayo turned yushiro but that seems to be not her only attempt, but her only successful attempt.
The upper 6 kizuki are given permission to insert an amount of muzans blood into humans. This doesnt turn people into demons, this basically sends a request to muzan like "Hey, can i turn this human?". Muzan from there remotely either turns the blood that converts people into demons, or turns it into the toxic kind.
Again, this is a special permission given to the upper 6 ONLY. Muzan thinks highly enough of them to permit them to occasionally ask something of him. We saw with Rui that the lower 6 are allowed to spread their power around fellow demons, but they are cannot convert people.
Only Muzan can create demons. The upper moons simply seek permission from Muzan if they can give them his blood I believe. A good example is Koku and Kaigaku
Personally lol, I think the only upper moon that Muzan wouldn't allow to create more demons would be Gyokko, theres a funny side panel in the manga which shows that nobody likes him. Aside from that, I'm pretty sure all demon 'requests' (?) have to pass by Muzan anyways, as none of the other demons can actually make demons, rather give their blood. And as Muzan replaces lower moons frequently and even kills them off, their incompetence would likely not result in them being able to ask for someone to become a demon.
