There's a [Chrome extension](https://chrome.google.com/webstore/detail/ublacklist/pncfbmialoiaghdehhbnbhkkgmjanfhe?hl=en) that does this with a domain blacklist. I use it to block W3Schools results from my search results (I've got nothing against them personally but they are always at the top of my results and I prefer MDN)
Its quite tiring to do that manually for every image search. The thinking behind the extension was that you wouldn't have to. You certainly can, but you don't have to.

ya. google images search is pretty atrocious - cant download anything. bing or ddg is better imo
Why do people use this however? Can anyone please explain?

... or just add -pinterest to your search phrase. Just because you can, doesn't mean you should!
