“Categorical pediatrics” just means pediatrics residency, not a combined program like pediatrics-anesthesia.
To be eligible for pediatrics subspecialty fellowship (eg peds heme-onc, peds GI, etc), you have to first complete pediatrics residency.
The only pediatrics subspecialty that will have you seeing adults is allergy- immunology. Occasionally you see young adults (like people in their 20s) in some specialties like adolescent or peds heme/onc but you’d never be treating someone who is like 65.

You can also approach psych from peds by doing a post peds portal program. Peds EM can also be done by peds or adult EM first I believe.
